Here’s our step by step guide to completing your enrolment so you can start to book care.
We recommend reading this before starting to fill out your enrolment form.
Step 1
Get all your information and documents ready
Before you fill out the form, get all the information and documents you need ready including:
Centrelink Customer Reference Number
To claim Child Care Subsidy (CCS), you will need to provide the individual Centrelink Customer Reference Numbers (CRNs) for both the primary carer and each child you are enrolling.
Please note, the primary carer listed on the enrolment form must be the same primary carer listed in Centrelink. If you are not sure, please check with Centrelink.
Payment information - bank account, debit or credit card details
You will need to provide your bank account or card details for direct debit. You are only charged for sessions already attended.
Emergency contact information
Please list at least one additional emergency contact in addition to your child’s guardian(s). Emergency contacts must over 18 years of age. Emergency contacts will also have permission to collect your child from OSCH.
Medical and other support documentation:
If your child has medical or other support needs, please provide the relevant documents such as:
- Medical Management Plan (MMP) – must be signed by a medical practitioner and dated within the last 12 months.
- Action Plan – for health conditions such as Anaphylaxis, Asthma, Epilepsy, or Diabetes.
- Details of any parenting or court orders in place
- A photo of your child to accompany any of the documents above.
These documents are mandatory to ensure we can support your child safely.
Step 2
Find your school’s OSHC enrolment form
Visit our locations page to find your school and your unique enrolment form.
This page also has the key information about your OSCH, including session times and fees.
Please note you will need to complete a separate enrolment form for each child who needs OSHC.
Step 3
Get set up and download the Xplor Home app
We will process your enrolment and send you a confirmation email.
You will also receive a link to download the Xplore Home app. This is our bookings and communication app.
